A copy of the amended well permit is <br /> enclosed. <br /> Once the well is constructed and the permanent pump is installed, well <br /> completion and pump installation reports including well and permanent pump <br /> test data must be submitted to this office. After water from the well has <br /> been put to use, evidence of beneficial use must be submitted to this office. <br /> A Statement of Beneficial 4.1Se form was provided to the applicant when the <br /> well permit was issued.
