<br />Shell Frontier Oil & Gas Inc. (SFOGI) hereby submits a map for the 2014 Annual Report for the 2012 -014
<br />MRP Public Land Discharges to Groundwater, NOI P- 2012 -014. The original report was submitted on July
<br />22nd 2014.
<br />It was brought to our attention, in correspondence from DRMS dated December 18, 2014, that an Annual
<br />Report Map was not included in the 2014 annual report materials submitted to DRMS on July 22, 2014 for
<br />NOI #P- 2012 -014. In that submittal and all prior annual report submittals, Shell indicated in the cover
<br />letter that general location maps, site maps, and financial warranty information for each of the sites
<br />covered under the groundwater discharge NOI can be found in the previously approved general
<br />prospecting NOI files maintained by DRMS, e.g. the general location map, site maps, and financial
<br />warranty information for drill pad W1 -14 can be found in the DRMS file for general prospecting NOI #P-
<br />2003 -015. Because of the specialized nature of NOI #P- 2012 -014 which covers only groundwater
<br />discharges occurring on drill pads previously permitted by DRMS in other NOI's, Shell assumed that
<br />referencing each of the existing general prospecting NOI's in the annual report cover letter would suffice
<br />for annual reporting purposes.
<br />The annual report map attached to this cover letter for groundwater discharge NOI #P- 2012 -014 is a
<br />regional location map showing the location of all drill pads covered under the NOI.
<br />For as -built hole data, general location map, site map(s), and financial warranty cost summaries please
<br />refer to the specific DRMS prospecting NOI files listed below (items in red have been removed from the
<br />NOI's by permit modification in the last reporting year):
<br />P- 2003 -015:
<br />Pads W1 -14, PW1 -28, W1 -28, W2 -28, W3 -28, 57 -15, 60 -21, 61 -22 and 62 -23.
