4 (FORM 2 - Public filing)
11. Describe proposed surface excavation or other land disturbance, including roads, pits, trenches, waste piles, drill pads and
collar areas of underground workings, ponds, etc...
NOI File No. P- 2009 -025 MD -02 is approved for up to 50 prospecting drill holes in the project area with necessary
access roads. As ot this submittal, 16 holes have been completed of which are DWK permitted groundwater
m itoring wells New dust, irbance is proposed forn ha ilk sample collection Vin hydral flor nq VP n Of rock
an ally jet-cl itti - a ca r
from within a standard bore hole. Surface disturbance for this operation will be in compliance with our currently
bonded amount, and the underground bulk sample cavity from the proposed activity will be backf1lied accordingly.
12. Proposed Disturbance (approximate) Describe the proposed drilling to be conducted, including anticipated number of holes,
diameter, depth, location, etc... Submit additional pages if necessary:
A. Drill Pads: Quantity 50.00 Average Width 30.00 (ft) Average Length 80.00 ( ft)
B. Drill Holes: Quantity 50.00 Depth 650.00 (ft) Diameter 12.00 (in)
C. Mud Pits: Quantity 50.00 Average Width 5.00 (ft) Average Length 15.00 (ft)
Average Depth 8.00 (ft)
Described proposed underground work, including reopening of old workings, advancement of adits or shafts,
trenches, pits, cuts, rock dumps, or other types of disturbance, describe type, quantity and general dimensions:
Upon completing bore hole into the target zone (shaft) a bulk sample collection
mining tool will be lowered to a target depth. The tool will use a water stream to cut
rock away from the bore hole annulus and also pump the cut material tot the surface
via the drill pipe string.
F. Other Disturbances (please describe):
Where possible, installation of groundwater monitoring wells will be in clusters. The clusters would
typically range from 2 -4 but up to 6 wells per drill pad location. Within the clusters, wells could be
located within 16'to 20'uf une another The anticipated distuibance wea foi each cluster could be
approximately 50' by 150' A 180' by 50' area is anticipated for the bulk sample
G. Indicate Chemicals and Fuels used or stored on site. List type, quantity and method to store.
pallets under cover from weather.
F. New Road(s): Length 1,500.00 (ft) Width 12.00 (ft)
Significantly Upgraded Road(s) Length 0.00 (ft) Width 0.00 (ft)
Are culverts or other crossings proposed? If so, please describe:
No crossings or culverts are currently planned, but if any are required, we will work with
the local surface owner and a quailified contractor to build any.
