PERMIT#:M-2013-018 <br /> INSPECTOR'S INITIALS:PSH <br /> INSPECTION DATE:December 17,2014 <br /> OBSERVATIONS <br /> This was a conversion application (CN-01) pre-operational inspection of the Harms Pit conducted by Peter <br /> Hays with the Division of Reclamation, Mining and Safety (Division/DRMS). Mr. Lonny Graves with Pine Bluffs <br /> Gravel & Excavating, Inc. was present during the inspection. <br /> The 110c to 112c construction materials permit conversion application was received by the Division on <br /> November 10, 2014. The application was deemed complete on November 21, 2014. The decision due date is <br /> February 19, 2014. The Applicant published public notice as required by Rule 1.6.2(d)for four consecutive <br /> weeks. The last notice was published on December 18, 2014, based on comments by the Operator. The public <br /> comment period for the application will end on January 8, 2015. No objections have been received by the <br /> Division to date. The notice sign was observed during the inspection as required by Rule 1.6.2(b). The Division <br /> mailed the first adequacy review letter for the application package on December 24, 2014. <br /> The conversion application will increase the permitted area from 9.98 acres to 32.2 acres as required by MV- <br /> 2014-024 and the Mined Land Reclamation Board Order effective September 3, 2014. The conversion permit <br /> area includes the access road from WCR 127 to the western permit boundary. As required by the Board <br /> Order,the Operator shall cease and desist any further activity on lands affected outside the approved permit <br /> boundary, with the exception of environmental maintenance, reclamation, or corrective actions until the <br /> Operator complies with the corrective actions and pays the civil penalty imposed by the Order. The <br /> conversion application is required to be in approvable form by March 2, 2015 as part of the corrective actions. <br /> The site was inactive at the time of the inspection. No mining equipment or additional mining disturbance <br /> since the previous inspection was observed. The four corner boundary markers for the conversion area were <br /> installed and marked with survey pins and wooden stakes at the time of the inspection. The conversion area <br /> consisted of cropland with evidence of a recent dry-land wheat crop. <br /> The proposed post-mining land use for the site is rangeland. The slopes will be reclaimed at a 3H:1V slope <br /> with 6"to 12" of topsoil replaced during reclamation. <br /> Pre-operational photographs taken during the inspection are attached.
