PERMIT#:M-2007-012 <br /> INSPECTOR'S INITIALS:ERR <br /> INSPECTION DATE: September 15,2014 <br /> OBSERVATIONS <br /> The Division of Reclamation, Mining and Safety(Division)conducted an inspection of the Larson Pit,Permit File No. M- <br /> 2007-012. The mine is located approximately 7 miles south-southeast of Haxtun,Colorado. Elliott Russell, of the <br /> Division,inspected the site while Gregory Larson accompanied it.The site is an active 112c operation with 68 permitted <br /> acres,with a maximum allowed disturbance of 33 acres. The approved post-mining land use is rangeland. This inspection <br /> was carried out as a part of the Division's normal monitoring program. <br /> At 9:45,the Division met Mr. Larson at the mine site entrance located 1/4 mile south of CR 16,on CR 7.At the time of the <br /> inspection it was chilly,the weather was overcast,and the ground was damp. The mine site identification sign was <br /> observed in the northwest corner of the permit boundary, and was in compliance with Rule 3.1.12(1). The permit <br /> boundary was fenced on all sides. <br /> The site is located within the flow area of the ephemeral channel known as Sandy Creek; at the time of the inspection this <br /> creek was not flowing,but the site did contain some standing water from recent rain events. Although one front-end <br /> loader was on the site at the time of the inspection,the site did not appear to have any recent mining activity. There was <br /> one large mined area on the south side of the creek located on the inside of a meander. This area was approximately 15 <br /> feet deep. Topsoil had been stripped and piled adjacent to this excavation on the east side. <br /> The maximum allowed disturbance is 33 acres and the Operator has been approved for only Area 1 of the mining plan <br /> (attached to this report is the Mine Plan Map submitted in 2007). Though the current disturbance is much smaller than <br /> what is approved,it is located in Area 1 and Area 2 with most of the disturbance occurring in Area 2. This will be cited as <br /> a problem requiring corrective action. The Operator will be required to submit a Technical Revision to adjust the Mining <br /> and Reclamation Plans. This TR shall include the request to be either approved for both Area 1 and Area 2 or the Operator <br /> can revise the current phasing to have a new Area 1,which includes at a minimum the current disturbance. If the second <br /> option is chosen,a new Mine Plan Map showing the new phasing will need to be submitted with the TR. <br /> A large pile,approximately 750 yards,of concrete rubble was observed along the haul road in Area 1 in the eastern <br /> portion of the permit. The Operator stated that this material was imported to the site and was going to be reprocessed as <br /> additional product. This material will either need to be hauled off-site or the importing/processing of recycled concrete <br /> should be included as a part of the Technical Revision mentioned above. A request to allow this processing within the <br /> approved permit,the location where processing shall occur,and the anticipated maximum volume of imported material <br /> should all be included in the TR. <br /> Photographs taken during the inspection are attached. <br /> Page 2 of 5 <br />
