On page 2 of the application,the responses to the questions numbered 6 and 7 do not match the persons listed <br /> in Exhibit O. Please correct this by listing the same individuals or stating"refer to Exhibit O"and resubmit <br /> page 2. <br /> 6.4 SPECIFIC EXHIBIT REQUIREMENTS—REGULAR 112 OPERATIONS <br /> The following items must be addressed by the Applicant in order to satisfy the requirements of C.R.S. 34-32.5- <br /> 101 et seq. and the Mineral Rules and Regulations of the Mined Land Reclamation Board: <br /> 6.4.3 Exhibit C—Pre-mining and Mining Plan Map(s) of Affected Lands <br /> 6. The Soil Resource Report Map does not include all soil units for the proposed area.Please resubmit the map and <br /> report,if necessary,showing a larger data window to include all areas of the new permit boundary. <br /> 7. Under the requirements of Rule 1.11.2(3)(b),please submit a map showing the area mined or disturbed and the <br /> area reclaimed since the original application. Though not specifically required under Exhibit C,the Division <br /> recommends submitting it under this exhibit to help define the pre-amendment conditions. <br /> 6.4.4 Exhibit D—Minim!Plan <br /> 8. The Exhibit D Mining Plan Map needs to show the new permit boundary as well as the original. In addition, <br /> this map should also indicate where current mining has occurred. <br /> 6.4.5 Exhibit E—Reclamation Plan <br /> 9. The Reclamation Plan states that the small addition to the north has stockpiled material from a previous <br /> disturbance and would be left as is,but would be seeded for reclamation. During the July 9,2014 site <br /> inspection,it was apparent that this area was re-disturbed by the current operation. Please commit to <br /> reclaiming this area with the same standards outlined in the Reclamation Plan for the rest of the site. <br /> 10. In the paragraph titled Revegetation Program in the attached 2008 Exhibit E Reclamation Plan,it discusses <br /> that the program was devised from review of the NRCS report.The Division believes the suggested <br /> reclamation seed mixture has too few species. Though the total seed rate of 62.7 f seeds/sgft/pls is <br /> appropriate,having 80%of it Smooth Brome suggests that the reclaimed area will not have an effective and <br /> diverse vegetative cover. Please submit the NRCS report that indicates this is recommended or acquire a new <br /> recommendation and adjust the plan as necessary. <br /> 11. In the paragraph titled Grasses in the attached 2008 Exhibit E Reclamation Plan,it states that if the seed is <br /> broadcast the amount will be increased by 75%.The Division recommends doubling the seed rate for <br /> broadcast seeding. Please commit to increasing the seed rate by 100%if broadcast seeding is utilized. <br /> 12. In the paragraph titled Reclamation Plan in the attached 2008 Exhibit E Reclamation Plan, it states that there <br /> are sufficient amounts of topsoil so the mined area can be re-soiled to a maximum depth of 10 inches overall, <br /> but also states this will only be done if the material is available. The Applicant should replace topsoil to a <br /> depth that at least meets the existing topsoil depth. Please commit to replacing topsoil to a depth of 10 inches, <br /> or specify and demonstrate the anticipated minimum depth or range of depth for the reclaimed areas of the <br /> site. <br />
