PERMIT#: M-1993-050 <br /> INSPECTOR'S INITIALS: TC1 <br /> INSPECTION DATE:November 19,2014 <br /> OBSERVATIONS <br /> This inspection was initiated by an Acreage Release (AR) request of 16.12 acres received by the Division on <br /> September 24, 2014 and revised on October 15, 2014 to 15.007 acres to account for keeping the haul road in <br /> the permit (see Figure 1). The Operator (Raymond F. & Robert W. Dellacroce) was represented by Mr. Mike <br /> Ausburn (Pioneer Sand Co. Inc.) who was present for the inspection. The Dellacroce Pit is a 112c gravel mine. <br /> The entrance is located approximately 1 mile west of Interstate 25, off West Baptist Road (exit 158). <br /> Inspection: <br /> A Permit sign was displayed as required (see Photo 1). <br /> The east end of the site was walked to inspect the status of the area delineated on the map (see Figure 2) <br /> included in the release request. A problem was cited requiring the Operator to <br /> submit either an acreage or surety reduction for completed reclamation. <br /> • The anniversary date is February 22. Annual reports and fees are current through 2014. <br /> 1. The 2013 annual report indicated 32.17 acres had been affected. The 2014 annual report <br /> indicated only 5 acres have been affected, but that 33.67 acres have been backfilled and 16.17 <br /> acres have been seeded. <br /> 2. Based on the 2013 annual report and the request to release 15 acres (AR-01), approval of AR-01 <br /> will reduce the currently affected area to 17.17 acres. <br /> • The post-mine land use is rangeland. <br /> Current Stipulations: <br /> None. <br /> Page 2 of 6 <br />
