Compliance With Mine Plan <br />N - Support Facilities Not On -site <br />N - Other <br />N - Special Categories Of Mining <br />N - Processing Waste <br />N - Topsoil <br />COMMENTS <br />This was a partial inspection of the Williams Fork Mines, permit number C- 1981 -044, operated by Moffat County <br />Mining, LLC. (MCM). I, Jared Ebert of the Colorado Division of Reclamation, Mining and Safety conducted the <br />inspection. Mr. Brian Watterson with MCM accompanied me during the inspection. The weather was clear and <br />sunny. MCM had completed several maintenance items that were noted during the October 16, 2014 DRMS <br />inspection. This inspection was conducted to document the maintenance items that were completed and determine <br />what remains outstanding. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Below is a list of the maintenance items associated with the protection of the hydrologic balance and an update of <br />their status. <br />Items No. 1: Clear the brush from the emergency spillways of the sediment control impoundments in accordance <br />with Rule 4.05.9(2) and Rule 4.05.9(11). <br />According to Mr. Watterson, MCM hired a contractor to evaluate each of the sediment ponds and conduct <br />maintenance. The Division observed all of the sediment ponds at the site. MCM has removed notable brush from <br />the emergency spillways of Pond 9 -P7, Pond 9A -P4, Pond 9A -P 1 and the primary spillway of Pond 5A -P I. It <br />appears the contractor missed several of the ponds and or did not recognize the emergency spillways. Brush needs <br />to be cleared from the emergency spillways of Ponds 9A -P2 and SH -P2. A large sage brush has grown over the <br />valve of the dewatering device of Pond 9A -P3, and the emergency spillway has brush growing in it. This <br />maintenance item is still outstanding and will need to be completed. The Division recommends that MCM review <br />the configuration and design of each of the ponds to ensure they are still configured as they are approved and <br />familiarize any future contractors of the locations of the pond spillways. The pond designs are located in Exhibit <br />Number of Partial Inspection this Fiscal Year: 1 <br />Number of Complete Inspections this Fiscal Year: 2 <br />Page 2 of 5 <br />
