PERMIT #: M- 2004 -051 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: December 1, 2014 <br />inlet and outlet structures required as part of the final reclamation plan have not been installed. <br />The approved post mining land use for the site is Developed Water Resource. As stated above, the slurry wall <br />has been completed around Phase 3 — Pond 3. The approved reclamation plan states all reservoir slopes will <br />be reclaimed to at least a 3H:1V final grade. The northern and northwest slopes of Pond 3 have been <br />regraded. The slopes appeared to be graded to 3H:1V, however the slopes were not measured by the <br />Division. A large stockpile of overburden was observed in the northwestern corner of Pond 3 on an unmined <br />portion of the phase. Additional overburden and topsoil stockpiles were observed east and south of the Pond <br />3 disturbance area. The Operator stated the disturbed areas east of the Phase 3 slurry wall, except the <br />perimeter access road, was reseeded and would not be further disturbed by mining activity. <br />The Operator is required to provide the Division agreements with the City of Westminster to mine within 200 <br />feet of the City owned structure. The Division has not been provided the City of Westminster agreements. <br />The original farm house and other buildings were present during the inspection. Mining excavation is <br />approaching the structures, but is greater than 200 feet away. <br />During the permit application review process, the Operator committed to notify the Division if during <br />dewatering or after installation of the slurry wall, the relative groundwater elevation between two or more <br />monitoring wells differed from the seasonal baseline condition by more than 2 feet and to provide monthly <br />monitoring groundwater well data along with the annual report. The required monthly monitoring well data <br />was submitted with the 2014 Annual Report for the site. <br />Photographs taken during the inspection are attached. An inspection map based on the latest Google Earth <br />image is attached.
