OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Gen. Compliance With Mine Plan <br />PROBLEM /POSSIBLE VIOLATION: Problem: The Operator must reduce the amount of active mining face to me <br />the Mining Plan requirements or revise the Mining Plan to allow for more active face, which would result in an <br />increase in the required financial surety amount. <br />CORRECTIVE ACTIONS: The Operator shall submit documentation the amount of active face has been reduced <br />to the allowed 2,000 lineal feet or submit a Technical Revision, with the required $216 revision fee, to update <br />and clarify the Mining Plan and Reclamation Cost Estimate to reflect existing and proposed activities by the <br />corrective action date. <br />CORRECTIVE ACTION DUE DATE: 2/03/15 <br />INSPECTION TOPIC: Gen. Compliance With Mine Plan <br />PROBLEM /POSSIBLE VIOLATION: Problem: The Division did not receive the slurry wall specifications and quality <br />control procedures for Division review, in the form of a technical revision, prior to construction of the Pond 3 <br />slurry wall. <br />CORRECTIVE ACTIONS: The Operator must submit the required technical revision within 30 days to avoid a <br />possible violation. <br />CORRECTIVE ACTION DUE DATE: 1/05/15 <br />Page 1 of 5 <br />
