5. Sequence of Events during Discharge and Corrective Actions Taken <br />As the mine officials began the work day at approximately 7 AM on October 24, the reddish color of the <br />water was noticed in Sneffels Creek and was traced to the mine discharge. Further investigation in the <br />mine found that the sediment laden water was coming from the pile of crushed rock at the bottom of the <br />raise bore. See Map 1 for these locations. <br />Mine officials Clint Fletcher and U Trujillo immediately contacted Greg Lewicki, consulting permit <br />engineer and Dianna Stoopnikoff, Environmental Manager for Fortune Revenue Silver Mines and a <br />conference call was arranged. The sequence of events of that day are detailed in the following Table 1 <br />by Greg Lewicki. <br />Page 7 of 17 <br />
