PERMIT #: M- 1995 -008 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: November 19, 2014 <br />OBSERVATIONS <br />This inspection was requested by the Operator, Fremont Paving & Redi -Mix, Inc. as the result of a Surety <br />Reduction (SR) request based on the mine having completed reclamation grading in the Phase 2 pit area. The <br />request was called complete for filing by the Division on November 17, 2014. Fremont Paving was <br />represented during the inspection by Angela Bellantoni (Environmental Alternatives, Inc.) and Jodi Ricker <br />(Fremont Paving). The LA Methodist Pit site is located approximately one half mile north of Florence, on the <br />east side of Highway 67, in Fremont County. <br />Inspection: <br />The reclamation progress was observed from the northeast corner of Phase 2. Grading, growth media <br />placement, track - walking and mulching appeared to be complete. (see Photos 1 and 2). A rock check dam was <br />observed at the south end of the pit for sediment control (see Photo 3). <br />File Review Notes: <br />• The previous inspection was conducted 5/20/2014. No problems were cited <br />• The total permitted area is 5.0 acres, after the approval of acreage release AR -01. <br />• There is a $33,700.00 financial warranty (corporate surety) for the site. The SR request was for a bond <br />reduction to $5,000.00 as grading is complete in Phase 2. <br />• Annual fees and reports are up to date through 2014. <br />• The pre- and post -mine land use is rangeland. <br />Bond: <br />The current financial warranty is a $33,700.00 corporate surety. The Division estimates seeding costs to be <br />—$900 /acre for a total of $4,000. The remaining $1,000 of the requested $5,000 bond should be adequate for <br />equipment, mobilization and supplemental seeding. <br />Summary & Recommendations: <br />The Division will recommend approval of the surety reduction request. A $5,000 bond should be adequate for <br />the remaining reclamation activities. <br />Page 2 of 4 <br />
