PERMIT #: M- 1977 -342 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: November 4, 2014 <br />OBSERVATIONS <br />The Henderson Mine was inspected by Peter Hays with the Division of Reclamation, Mining and Safety <br />(Division) as part of the Division's monitoring inspection program. Mr. Miguel Hamarat, Mr. Tim Haynes and <br />Ms. Amber Parmet with Climax Molybdenum - Henderson Operations (Henderson) were present during the <br />inspection. <br />The inspection began with a meeting to discuss the ongoing permitting items at the site. The following topics <br />were discussed: the tailings delivery line structural support refurbishment project progress (TR -22 and TR -24), <br />the progress of the Ute Park extraction well electrical and piping infrastructure (TR -23), the acid leaching of <br />Molybdenum Concentrate from the Climax Mine at the Henderson Mill, bacterial test results at the Aspen <br />Canyon Ranch POC well, and status of the outstanding as -built drawings and engineering certification for TR- <br />22, TR -23 and TR -24. <br />Climax Concentrate leaching: <br />The process for the leaching of Molybdenum concentrate from the Climax Mine at the Henderson Mill was <br />explained while in the Henderson Mill. The bag splitter and the associated changes to the existing acid leach <br />circuit were explained during the inspection. The Division received a letter describing the process in more <br />detail on November 6, 2014. <br />Tailing Delivery Line (TDL) structural support refurbishment: <br />The TDL refurbishment project was observed from Drop Tower 1 to Drop Tower 12. The last 1200 feet of RCP <br />was scheduled to be replaced during the next mill shutdown the following week. Sections of the fused and <br />ready to be installed HDPE were observed near 3 -Dam during the inspection. <br />Tailing Delivery Line (TDL) leak follow up: <br />The location of the TDL leak which occurred on October 8, 2014 was inspected. Evidence was observed <br />outside and around the energy dispersing curtain on Drop Tower 5 and near Drop Tower 12. The path of the <br />spill was followed to the location where it entered the tailings impoundment. <br />Ute Park extraction well: <br />The electric service line, vault, pitless adaptor and discharge piping and outlet for MLEX -1 were nearing <br />completion. The final electrical panel at the vault and the required instrumentation was waiting completed. <br />Seep Water Return line relocation: <br />Henderson staff stated the possible removal of the hill between 1 -Dam and 3 -Dam was no longer being <br />considered to expedite the connection of the two dams into one continuous dam face. <br />Aspen Canyon Ranch POC well: <br />Henderson staff bacterial tests for the Aspen Canyon Ranch POC well were positive and a report of the tests <br />and findings would be sent to the Division in the future. <br />Page 2 of 5 <br />
