PERMIT #: P- 2008 -043 <br />INSPECTOR'S INITIALS: THM <br />INSPECTION DATE: October 22, 2014 <br />OBSERVATIONS <br />This inspection was conducted as part of the normal inspection program conducted by the Division of <br />Reclamation, Mining and Safety (DRMS). Richard Blubaugh of Powertech (USA) Inc. was present during the <br />inspection. The Centennial Uranium Project is an inactive is -situ uranium prospect that consists of a maximum <br />allowed disturbance of 2.90 acres. <br />The purpose of the inspection was to observe the conditions of the prospect location. Various hydrology <br />monitoring wells were observed during the inspection.
