This item is in <br />addition to the three items that were originally identified in the letter the Division sent your office dated <br />October 27, 2014. <br />According to the ground water monitoring plan in Exhibit 29 of the permit, Well 913F has been installed <br />and is required to be monitored on a quarterly basis within three months after the No. 9 Mine waste rock <br />backfill area has undergone final grading. Well 913F is required to be monitored for water levels and the <br />parameters listed on Table 8 of the monitoring plan. According to the 2013 Annual Reclamation Report <br />(ARR) and confirmed in the 2009 ARR, the No. 9 Portal Area and associated refuse disposal area was <br />reclaimed in 2009. Given this, water monitoring should have been initiated at this well while the site was <br />in active status. Based on my review of the Division's files the mine was placed back into active status <br />during the 3rd Quarter of 2010 and continued until the 3rd quarter of 2013.
