Mr.Michael Cunningham 2
CDRMS
October 27,2014
Subsequently, on 28 May 2014 UMC submitted a proposal ("Leadville Mill, M1990-057,
Proposed Monitoring Well Locations"). The proposal was to construct Monitoring Well (MW)
No. 2 as the down-gradient well at the extreme southwest corner of the property, and MW-3 as
the up-gradient well northeast of the TSF near our lower mill access road. Locations are shown
on our updated Exhibit 6-2 also provided in the 281h May Proposal.
UMC received a verbal "OK" accepting the proposal; however no formal letter was received by
UMC.
UMC, applied and obtained permits from the State Engineer (Permit 295655 for MW-2, and
295654 for MW-3) on 11 September 2014. Copies of the permits are provided in Attachment
No. 2. We then secured a drilling contractor who completed the work on 14 October 2014.
UMC completed the monitoring well project on 24 October 2014 by installing well pumps.
UMC will collect and submit for testing its first water samples from MW-2 and MW-3 during the
fourth quarter of 2014. Results will be presented to the Division in our annual reports.
Photos of MW-2 and MW-3 are provided below.
MW-2, Down-Gradient Well MW-3, Up-Gradient Well
Exhibit C, Construction Schedule
Delays associated with the final construction and operation of the mill continues to be influenced
by poor metals market conditions. However, UMC is in discussion with two (2) potential
investor/funding partners. Our goal is to complete these negotiations with one of these possible
funding avenues and complete construction of the TSF during Q2 2015. Our current schedule
to complete the TSF is:
Activity Date Duration Inspection
General Site Preparation May 18 - May 19 2 days No Inspection
Finalize Embankment Construction May 20 — May 26 5 days B CDRMS
Geos nthetic Clay Liner Installation May 27 —Jun 4 7 days B CDRMS
Impoundment Liner Installation May 27—Jun 4 7 days Incl. with above
Slope Ballast Installation Jun 8—Jun 12 5 days CDRMS Optional
Tailings Slurry Piping, Decant Water Pumps Jun 8 —Jun 12 5 days No Inspection
Historic Tailings Placement Jun 15 —Jun 26 10 days CDRMS Optional
Note: Schedule assumes 5 working days per week.
