Please add the name and address for Joseph Dugan Revocable Trust to <br />the surface and coal ownership list provided on revised page 2.03 -8. <br />This entity is depicted on revised Map 1 but is not listed on the revised <br />text. <br />ii. Please add the following names and addresses to the coal ownership list <br />on revised paged 2.03 -9 to 10: Edna Paulsen and Florence Vantaseel, <br />Betty Rodgers and Bernice Glasburn. These entities are depicted on the <br />revised Map 2 but are not included in the revised text. <br />Response: The Joseph Dugan Trust has been added to the surface and coal <br />ownership list. MCM contacted the Moffat County Assessor to verify the <br />mineral ownership of the lands in question under Item 3(d)(ii) above. The <br />Assessor's Office responded that the Federal Government is the mineral (coal) <br />owner for all of the identified tracts. Map 2 has been revised and updated to <br />reflect the corrected mineral ownership information. <br />Rule 2.03.8 — Permit Term Information <br />2) Revised Map 24 scale is too small to read the dates and other information provided on <br />the map image. Also, the vertical extent of the proposed underground mine working are <br />not visible or not depicted on the map. Please increase the scale of Map 24 so the <br />required information is visible and indicate the vertical extent of the underground mine <br />workings. <br />a. MCM Response to items 1 and 2: Page 2.03 -19 has been revised to include <br />disturbed and affected acreage information, and the revised page is included <br />with these responses. Map 24 has been revised to delete the information on <br />mining dates for areas already mined and add coal seam elevation data. <br />b. DRMS Response #1 <br />i. Item #1 is resolved <br />ii. Item #2: Rule 2.03.8 for underground mining activities, requires that the <br />applicant state the anticipated and actual starting date and termination <br />date of each phase of the underground mining activities. Please revise <br />Map 24 to indicate the actual starting and termination date for each phase <br />of the underground mining operation. <br />c. MCM Response: <br />i. Maps 23 and 24 have been revised to note time intervals for mining <br />phases. <br />d. DRMS Response: This item is resolved. <br />Rule 2.03.9 — Personal Iniury and Property Damage Insurance <br />1) Please revise Exhibit 1 with the most recent insurance certificate. <br />
