The Responsibilities as a Permittee section of the Application Form on Page 5 and 6 only contains <br /> one set of initials. Since the permit will be under the names of Mr.Anderson and Ms.Adkins,both <br /> sets of initials are required.Please submit replacement pages with both sets of initials or remove Mr. <br /> Anderson's name from item No. I on the first page of the application form. <br /> 6.3.1 Exhibit A—Legal Description and Location Map <br /> 2. A legal description and location map was not included with the application. The legal description must <br /> identify the affected land, specify affected areas and be adequate to field locate the property. <br /> Description shall be by township, range, and section, to at least the nearest quarter-quarter section. A <br /> metes and bounds survey description is also acceptable. Also, the street address or lot number(s) may <br /> be used. This information is available from the County Assessor's office or U.S. Geological Survey <br /> (USGS)maps. <br /> 3. A map showing information sufficient to determine the location of the affected land on the ground and <br /> existing and proposed roads or access routes to be used in connection with the mining operation. <br /> Names of all immediately adjacent surface owners of record shall also be shown. The operation <br /> location map shall be a standard 1:24,000 scale U.S. Geological Survey map. The location of the <br /> proposed operation shall be shown and labeled with the mine site name. <br /> 6.3.9 Exhibit I—Proof of Filing with County Clerk <br /> 4.
