2. A check valve will be installed on the end of the fresh water fill pipe above ground <br /> and readily observable to further insure that no future backflow can occur. <br /> 3. A highly visible sign will be affixed to the above ground valve handle immediately <br /> adjacent to the mine entrance road so that when the valve is in the pond fill position it <br /> will read `Pond' and when in the normal process plant position, it will read `Plant'. <br /> 4. All current employees will be re-informed and re-trained on the correct operation of <br /> the Pond systems and valves and will be reminded at least twice a month during normal <br /> mine safety morning safety `tail gate safety meetings'. Proper pond operation will be <br /> reinforced and expanded during new employee safety and procedure training. <br /> 5. Previously planned sediment clean out will immediately commence to remove the <br /> sediment from Pond 3 and 4. Pond 4 will not be allowed to fill with sediment again via <br /> continuous removal from not only Pond 1 and 2 but also 3. <br /> The above plan will not be implemented until approval and/or modification by DRMS <br /> and operations will not resume until granted permission by DRMS. <br /> Obviously HMMC wishes to proceed immediately, so it is hoped that this plan will get <br /> immediate review, comment, change if required, and approval, so that the corrective <br /> actions can be taken. <br /> 4 <br />
