PERMIT#: M-1977-342 <br /> INSPECTOR'S INITIALS: PSH <br /> INSPECTION DATE: September 25,2014 <br /> OBSERVATIONS <br /> The underground facilities at the Henderson Mine were toured by the Division of Reclamation, Mining and <br /> Safety (Division). The following Division staff members were present on the tour: Peter Hays, Elliott Russell, <br /> Amy Eschberger and Tyler O'Donnell. The tour included observation of the underground facilities on the <br /> production level, crusher and truck level, truck maintenance shop, PC1/PC2 transfer station and the <br /> locomotive maintenance shop. <br /> Following the underground tour, the above ground mine support facilities including the hoist building were <br /> observed by the Division. <br /> The newly installed monitoring wells (MNGW-3 and MNGW-4) located west and north of MNGW-1 were <br /> observed. The wells were completed the day before this inspection to approximately 20 feet and 24 feet in <br /> depth respectively.
