CMS 62167 <br /> EXHIBIT G—INSURANCE <br /> Lessee shall obtain and maintain insurance coverage of the following types and amounts,subject to the terms <br /> indicated, as described below. <br /> 1. Worker's Compensation <br /> Worker's Compensation Insurance as required by state statute,and Employer's Liability Insurance <br /> covering all of Lessee and Lessee Subcontractor employees acting within the course and scope of their <br /> employment, with a minimum limit of$1,000,000. <br /> 2. Commercial General Liability <br /> Commercial General Liability Insurance written on ISO occurrence form CG 00 01 10/93 or <br /> equivalent, covering premises operations, fire damage,independent contractors,products and <br /> completed operations,blanket contractual liability,personal injury,and advertising liability with <br /> minimum limits as follows: (i)$1,000,000 each occurrence;(ii)51,000,000 general aggregate;(iii) <br /> S 1,000,000 products and completed operations aggregate;and(iv)$250,000 any one fire. If any <br /> aggregate limit is reduced below$1,000,000 because of claims made or paid,Lessee or Lessee <br /> Subcontractor shall immediately obtain additional insurance to restore the full aggregate limit and <br /> furnish a certificate or other document satisfactory to the State showing compliance with this provision. <br /> 3. Commercial Automobile Liability <br /> Automobile Liability Insurance and must include an MCS 90 form,covering any auto(including <br /> owned,hired and non-owned autos)with a minimum limit of$1,000,000 each accident combined <br /> single limit. <br /> 4. Contractor's Pollution Liability <br /> Contractor's Pollution Liability Insurance covering"Commercial Operation"performed by or on <br /> Lessee's behalf. Coverage must be written on an occurrence basis with a minimum limit of$2,000,000 <br /> per occurrence and in the aggregate,including extensions of coverage for aunsportadon, including <br /> loading and unloading,and non-owned disposal sites(if applicable to the scope of work performed <br /> under the Lease). The policy shall not include,or shall remove,any exclusions for claims arising from <br /> real property owned,rented or leased by Lessee. If coverage is written on a claims-made basis, <br /> coverage must be maintained for a minimum of three(3)years following the completion of the Lease <br /> or work performed under the Agreement. <br /> 5. Property Insurance <br /> All Risk Property Insurance for Lessee's real and personal property and Lessee's equipment at the <br /> Leased Premises. <br /> 6. Umbrella or Excess Liability Insurance with minimum limits of 55,000,000 <br /> This policy shall become primary(drop down)in the event the primary Liability Policy limits are <br /> impaired or exhausted. The Policy shall be written on an Occurrence form and shall be following form <br /> of the primary. <br /> 7. Deductibles <br /> Lessee shall bear the sole responsibility for any deductibles. <br /> 9. Additional Insured <br /> The State shall be named as additional insured on all Commercial General Liability and Excess or <br /> Umbrella Liability Insurance policies(leases and construction contracts require additional insured <br /> coverage for completed operations on endorsements CG D3 16(07040)or equivalent)required of <br /> Lessee and any Lessee Subcontractors hereunder. <br /> 9. Primacy of Coverage <br /> Coverage required of Lessee and Lessee Subcontractors shall be primary over any insurance or self- <br /> insurance program carried by Lessee or the State. <br /> 10.
