September 24, 2014 <br />X- 2005- 109- 11 /Colowyo 2005 Reserve Def Drilling Program JDM <br />During the 2013 SL -1 bond release inspection, three 2" PVC pipes, approximately 8" tall, were located on LW -05- <br />08 and one was located on the east edge of LW- 05 -11. These required removal to achieve final bond release. <br />During this inspection no PVC remained at Site LW- 05 -11. PVC was identified at Site LW -05 -08 during the <br />inspection. Martin Stearns was advised to remove the PVC and submit before and after photographs of the work <br />completed. The Division received photo documentation of the removal of the PVC via email on September 30, <br />2014. <br />During the 2013 SL -1 bond release inspection, an open borehole was identified at Site LW- 05 -15. This hole was <br />located on the west side of the access road on Sherriff s Ridge, 10' north of the t -post marker for the pad. This hole <br />is approximately 15' deep and 8" in diameter and will need to be properly sealed to achieve final bond release. <br />During this inspection, the open hole location was identified and the hole was sealed. No evidence of slumping or <br />instability was identified. <br />Based on this inspection, the Division determines that the exploration activities associated with X- 2005 - 109 -11 <br />have been adequately reclaimed, except the monitoring well that remains at Site C- 05 -33. The Division <br />recommends the release of all other exploration activities associated with this NOI. <br />• 'd trf� <br />.
