The <br /> Division has approved this request and the decision date has been extended an additional 30 days. The Division is <br /> now required to make an approval or denial decision no later than October 27,2014; therefore,a response <br /> to the August 22,2014 adequacy review letter should be submitted to the Division as soon as possible.In <br /> order to allow the Division adequate time to review your responses,please submit your adequacy responses to the <br /> Division no later than one week prior to the decision date(October 20, 2014). <br /> If more time is needed to complete the reply,the Division can grant an extension to the decision date.This will be <br /> done upon receipt of a written waiver of the Applicant's right to a decision by October 27, 2014 and the request <br /> for additional time.
