Fortune Revenue Silver Mines Inc. Revenue Mine MCP Plan 2014
1.2 Professional Engineer Certification (40 CFR 112.3(d))
The undersigned Registered Professional Engineer is familiar with and has visited and examined the
facility, or has supervised examination of the facility by appropriately qualified personnel. The
undersigned Registered Professional Engineer attests that this Material Containment Plan has been
prepared in accordance with good engineering practice, including consideration of applicable industry
standards; that procedures for required inspections and testing have been established; and that this Plan
is adequate for the facility.
This certification in no way relieves the owner or operator of the facility of his/her duty to prepare and
fully implement this MCP Plan. This Plan is valid only to the extent that the facility owner or operator
maintains, tests, and inspects equipment, containment, and other devices as prescribed in this Plan.
1.2.1 Required Improvements
The Professional Engineer's certification of this plan is contingent on the following facility
improvements being implemented for compliance with MCP:
1) A full complement of emergency spill kit items must be placed near the reagent
room and outside diesel tanks for use on the entire site. These kits should include a
50+ gallon wheeled container with absorbent socks, pillows, pads, gloves, goggles,
PPE, disposal bags, ties, manual and labels.
2) Training of personnel must occur per Section 3.7 of this plan and designated
employees must become familiar with the spill kit items, their use and proper
reporting in the case of a spill.
Signature.
Name: Greg Lewicki
State and Registration #:
20335
Company: Greg Lewicki and Associates
Date: 8/28/14
4
