Fortune Revenue Silver Mines Inc. Revenue Mine MCP Plan 2014 <br />3.6.2 Monthly Inspection <br />The checklist provided is used for monthly inspections by Star Mine Operations personnel. The monthly <br />inspections cover the following key elements: <br />• Observing the exterior of aboveground storage tanks, pipes, containers, and other equipment for <br />signs of deterioration, leaks, corrosion, and thinning. <br />• Observing the exterior of portable containers for signs of deterioration or leaks. <br />• Observing tank foundations and supports for signs of instability or excessive settlement. <br />• Observing the tank fill and spill/release pipes for signs of poor connection that could cause a <br />spill/release, and tank vent for obstructions and proper operation. <br />• Verifying the proper functioning of overfill prevention systems. <br />• Checking the inventory of spill /release response equipment and restocking as needed. <br />• Check all types of secondary containment (vertical walls and epoxy coatings) on site for damage. <br />All problems regarding containers, tanks, piping, containment, or response equipment must immediately <br />be reported to the Facility Manager. Visible leaks from tank walls, piping, or other components must be <br />repaired as soon as possible to prevent a larger spill or a spill /release to navigable waters or adjoining <br />shorelines. Pooled liquid is removed immediately upon discovery. The Monthly Inspection Checklist is <br />included in this section and the inspection will be conducted during normal seasonal operation of the <br />site. <br />Written monthly inspection records are signed by the Facility Manager and maintained with this MCP <br />Plan for a period of five years. <br />15 <br />
