Reclamation Plan Map: See attached maps page 1 and 2 <br />The operator has included 2 pages including maps describing features associated with the site, <br />mining plan, and reclamation plan. The map(s) are separated and identified as: <br />Page 1— Vicinity Map. An aerial view indicating the proposed mining location, nearest town <br />(Wiley, CO.) 2.4 miles to the Northeast, Property Description and date illustrated. <br />Page 2 - Exhibit A, B, C, D. Map Legend, Site description, North Arrows, Adjacent Property <br />Owner, surrounding land features and date illustrated. <br />a. The anticipated gradient is shown on page 2 of the plan identified as Exhibit D. <br />b. Not apalicabic The reclamation plan does not include shrub and tree planting, therefore, the <br />plan does not identify any area where vegetation will not be established. <br />c. Not apdicable. There are no ponds included in the reclamation plan. <br />d. The average thickness of overburden replaced is 4' - 8 ". <br />e. The average thickness of topsoil replaced is 8" as shown on page 2 of the plan identified as <br />Exhibit D. <br />6.3.6 EXHIBIT F- List of other permits and licenses required <br />Odher permits and Licenses: <br />Upon approval by the State of Colorado Division of Reclamation, Mining and Safety the operator <br />shall secure a Certification to Discharge under CDPS General Permit COR34 -0000 Storm water <br />Discharges Associated with Sand and Gravel Mining and Processing. <br />6.3.7 EXHIBIT 6- Source of lertai riAlhtto -enter <br />Legal right -to enter: <br />Attached is a letter provided by Hans Friederichs and dated July 1, 2014 authorizing <br />Riverside Aggregates, LLC the right-to-enter and seek permits for mining the permitted property. <br />6.3.9 EXHIBIT I — Proof of filinrt with the county clerk <br />Proof of filing: <br />The permit application has been filed with the Prowers County Clerk. Attached is the proof of filing. <br />6.3.10 EXHIBIT J — Proof of mailinat of notices to Board of County Commissioners and Soil Conservation <br />District. <br />Proof of delivering notices: <br />Notices to the Prowers County Board of Commissioners and Prowers County Soil Conservation <br />District have been delivered.
