PERMIT #: M- 2004 -023 <br />INSPECTOR'S INITIALS: ERR <br />INSPECTION DATE: August 19, 2014 <br />OBSERVATIONS <br />The Division of Reclamation, Mining and Safety (Division) conducted a conversion application's pre- <br />operational inspection of the Haynes Hereford Ranch Pit, Permit File No. M- 2004 -023. The mine is located <br />approximately 3 miles west - northwest of Holyoke, Colorado. Elliott Russell, of the Division, inspected the site <br />and Randy Schafer, with Phillips County, accompanied it. <br />The 110c to 112c construction materials permit conversion application was received by the Division on June 3, <br />2014. The application was deemed complete on July 1, 2014. The decision due date is September 29, 2014. <br />The conversion application will increase the permitted area from 9.9 acres to 70.3 acres. The purpose of this <br />conversion is to include areas that were disturbed outside of the original 110c permit boundary as well as <br />additional undisturbed land for future mining. The proposed plan is to mine the creek banks of the North Fork <br />of Frenchman Creek, an ephemeral creek which meanders through the proposed permit boundary. The proposed <br />post mining land use for the conversion area is wildlife habitat, the same as the original 110c permit. <br />At 13:00, the Division and Mr. Schafer arrived at the mine site on the north side of CR 24, % mile east of CR <br />33. At the time of the inspection it was hot, the weather was clear, and the ground was dry. The posted notice <br />was observed during the inspection as required by Rule 1.6.2(b). The 110c mine site identification sign was also <br />observed at the entrance and was in compliance with Rule 3.1.12(1). The boundary for the new permit was <br />delineated by fence lines, except for the northwest corner; there was no fence or marker to represent the corner. <br />This will be cited as a problem and will need corrective action; please see page 1 of this report for more <br />information. The 110c mine site was active at the time of the inspection. There was a belly -dump haul truck <br />and a front -end loader on site. <br />During the inspection, the Division observed several structures that were not included in the conversion <br />amendment under Exhibit S. These included private buildings and fences located southwest of the permit on the <br />southwest corner of the intersection of County Roads 24 and 33. Also near this intersection, but within the <br />permit boundary, is a water spigot will also need a structure agreement. Additionally, an overhead utility line <br />was observed and an indicator that a fiber optic line was buried underground, both along the southern permit <br />boundary. The lack of these structures being mentioned in the application and the associated signed structure <br />agreements has been included in the adequacy review letter sent on September 3, 2014. <br />Photographs taken during the inspection are attached. <br />Page 2 of 7 <br />
