PERMIT #: M- 1978 -249 <br />INSPECTOR'S INITIALS: ERR <br />INSPECTION DATE: August 19, 2014 <br />OBSERVATIONS <br />The Division of Reclamation, Mining and Safety (Division) conducted an amendment application's pre- <br />operational inspection of the Kokes Gravel Pit, Permit File No. M- 1978 -249. The mine is located approximately <br />8 miles north of Crook, Colorado and is adjacent to the MMM Partnership Gravel Pit (M- 2006 -042). Elliott <br />Russell, of the Division, inspected the site while Chad Wright, with Logan County Road & Bridge, <br />accompanied it. <br />The 112c construction materials permit amendment application was received by the Division on June 5, 2014. <br />The application was deemed complete on June 27, 2014. The decision due date is September 25, 2014. <br />The amendment application will increase the permitted area from 20.1 acres to 32.25 acres. The purpose of this <br />amendment is to include areas that were disturbed outside of the original 112c permit boundary as well as <br />additional undisturbed land to allow for reclamation sloping. The proposed plan is to only reclaim the site and <br />the Applicant has stated that no mining will occur. The proposed post mining land use for the amendment area <br />is rangeland, the same as the original 112c area. <br />At 09:15, the Division met Mr. Wright at the mine site on the east side of CR 79, '/ mile south of CR 76. At the <br />time of the inspection it was hot, the weather was clear, and the ground was dry. The posted notice was <br />observed during the inspection as required by Rule 1.6.2(b). The 112c mine site identification sign was also <br />observed at the entrance and was in compliance with Rule 3.1.12(1). The boundary markers for the new permit <br />were installed; these markers were t -posts painted orange. The 112c mine site was not active at the time of the <br />inspection and no equipment was on site. <br />The mine site is located on the side of a small bluff which slopes to the northeast. During the inspection, it was <br />discussed that there could be a potential for offsite impacts because the mine site slopes into a dry creek <br />drainage on the northeast side; sediment from the disturbance area could end up in this drainage.
