The <br />Division has determined that this request must be submitted as a Technical Revision to be accompanied by <br />the required $216 fee. Furthermore, such a request must also be submitted by the Permittee or the Permittee <br />must provide the Division with a letter authorizing Arcadis, in this case to submit a revision on the behalf of <br />Holcim. <br />Attached is a "Request For Technical Revision (TR) Cover Sheet" for your convenience.
