OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Hydrologic Balance and Offsite impacts <br />PROBLEM: Alluvial groundwater is mounding against the slurry wall at the Stagecoach Pit, causing an elevated <br />groundwater table up gradient from the slurry wall. Significant constrictions in alluvial groundwater flow have <br />resulted in elevated lake levels in Zigan Lake. This is a problem for failure to minimize the disturbance to the <br />prevailing hydrologic balance pursuant to C.R.S. 34- 32.5116(4)(h) and for failure to protect areas outside of the <br />affected land from slides or damages occurring during the mining operation pursuant to C.R.S. 34- 32.5 - 116(4)(1). <br />CORRECTIVE ACTIONS: Submit to DRMS by September 22, 2014 a groundwater mounding mitigation plan <br />as either a Technical Revision or Amendment to the permit. The plan must include: <br />1. A detailed description of the measures the operator will take to reduce groundwater mounding outside the <br />Stagecoach Pit to a level that eliminates elevated levels in Zigan Lake, <br />2. A time schedule for implementing the plan, and <br />3. Enough information for DRMS to estimate the cost of implementing the plan (for establishing the amount of <br />required financial warranty). <br />CORRECTIVE ACTION DUE DATE: 9/22/14 <br />Page 1 of 7 <br />
