PERMIT #: M -1993 -050 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: January 28, 2014 <br />Bond: <br />The current bond is inadequate for 33.67 acres of affected area. The Operator must request a surety reduction <br />for reclamation performed or an acreage release for reclamation completed on affected area where there is to be <br />no more mining in an approvable form by the due date on page 1 of this report). If neither of the required <br />requests are received by the due date, the DRMS will request a surety increase for the entire 33.67 acres. <br />Summary and Recommendations: <br />1. The Operator needs to repair the erosion gullies, if not already done so. <br />2. The Operator needs to put the permit sign up, if not already done so. <br />3. The Operator should clean out the sediment pond, if not already done so. <br />4. Within 30 days of this report, the Operator must request either: <br />a. A surety reduction for reclamation performed, or <br />b. An acreage release for reclamation completed on affected area where there is to be no more <br />mining within 30 days of this report. <br />PHOTOGRAPHS <br />Photo 1.
