August 28, 2014 C- 1981 - 033 /13ear Mine SLB <br />Surfacing and Maintenance4.03.1(5) and (6)/4.03.2(5) and (6) <br />Reclamation 4.03.1(7)/4.03.2(7): <br />The pavement on the access road to the site has begun to deteriorate; cracks have developed in the surface, <br />and vegetation is pushing up along the roadway edges. The bridge over the North Fork of the Gunnison river is <br />also in very poor condition, with rotting decking. The embankments abutting the bridge are eroding such that <br />there are large gaps between the bridge and the roadway on both ends of the bridge. <br />SLIDES and DAMAGE - Rule 4.12: <br />Evidence of hillside creep is apparent in the slide area on the west end of the reclaimed portal faceup. The <br />material at the base of the slide face is consolidating, exposing additional highwall along the slide scarp.
