No <br />representative of C.B. Minerals Company, LLC was present during the inspection. Records were inspected at the <br />Garfiled County Courthouse at 11:30 am; the site inspection began at 12:08 pm. Weather conditions were sunny <br />and warm; ground conditions were muddy from rain earlier in the day. Cattle were grazing the reclaimed <br />disturbance area. <br />The mine site disturbance has been reclaimed and Phase II bond release has been granted on the entire site. <br />Sediment control structures have not yet been reclaimed. <br />AVAILABILITY OF RECORDS — Rule 5.02.4(1): <br />Records were inspected at the Garfield County Courthouse. All required records were available, with the <br />exception of the updated insurance certificate. The certificate has been received at the DRMS Denver office, but <br />had not yet been filed at the courthouse. <br />FISH and WILDLIFE — Rule 4.18: <br />A grouse was flushed along the roadway through the site, between the middle and upper terraces. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7,4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Both cells of the sediment pond were dry, and there was no flow observed in any of the ditches or culverts. <br />ROADS — Rule 4.03 <br />Construction 4.03.1(3)/4.03.2(3) <br />Drainage 4.03.1(4)/4.03.2(4) <br />Number of Partial Inspection this Fiscal Year: 0 <br />Number of Complete Inspections this Fiscal Year: 1 <br />Page 2 of 7 <br />
