PERMIT #: M- 1983 -176 <br />INSPECTOR'S INITIALS: TOD <br />INSPECTION DATE: August 14, 2014 <br />CONCLUSION <br />Based on the groundwater elevations observed during the recent inspections and groundwater elevations <br />submitted following the inspection, DRMS believes the Operator has began to relieve upgradient groundwater <br />mounding. DRMS will continue to review groundwater monitoring data. <br />DRMS observed the construction of a new dewatering trench (the East Boundary Trench). This trench has not <br />been incorporated into the approved permit. The Operator shall submit a Technical Revision to incorporate the <br />East Boundary Trench into the permit. Accordingly, DRMS replaces Corrective Action 2a through 2d of the <br />August 4, 2014 inspection with the following corrective action: <br />2. Submit to DRMS a Technical Revision providing a revised groundwater mounding mitigation plan by <br />September 5, 2014. The revised plan shall include the following: <br />a. A revised design for the East Boundary Trench that is being excavated along the east side of the permit area. <br />The plan shall include a design to relieve groundwater mounding and indicate elevations of the bottom of the <br />trench along its length. <br />b. A demonstration that, in accordance with Rule 6.4.19, the Operator has valid structure agreements with the <br />owners of all structures located within 200 feet of the East Boundary Trench. <br />c. A commitment to submit a certification by a qualified engineer confirming the East Boundary Trench has <br />been constructed as designed. <br />d. A monitoring and maintenance plan for the East Boundary Trench, with bi- monthly reporting to DRMS of the <br />results of monitoring and maintenance activities. <br />e. A construction schedule for completing the construction of Phase 1 of the French drain. <br />One Technical Revision may be submitted for addressing the weed control plan (cited in the June 2, 2014 <br />inspection report), the East Boundary Trench, and the groundwater mounding mitigation plan. <br />Page 5 of 8 <br />
