The following inspection tonics were identified as having Problems or Possible Violations. OPERATORS
SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE
WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a
Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land
Reclamation Board will consider possible enforcement action.
INSPECTION TOPIC: General Compliance With Mine Plan
PROBLEM: The East Boundary Trench has not been incorporated into the permit.
CORRECTIVE ACTIONS: (This corrective action replaces corrective action 2a through 2d of the August 4,
2014 inspection report)
2. Submit to DRMS a Technical Revision providing a revised groundwater mounding mitigation plan
September 5, 2014. The revised plan shall include the following:
a. A revised design for the East Boundary Trench that is being excavated along the east side of the permit area.
The plan shall include a design to relieve groundwater mounding and indicate elevations of the bottom of the
trench along its length.
b. A demonstration that, in accordance with Rule 6.4.19, the Operator has valid structure agreements with the
owners of all structures located within 200 feet of the East Boundary Trench.
