PERMIT#: M-1977-342 <br /> INSPECTOR'S INITIALS: PSH <br /> INSPECTION DATE:August 12,2014 <br /> OBSERVATIONS <br /> The Henderson Mine was inspected by Peter Hays with the Division of Reclamation, Mining and Safety <br /> (Division) as part of the Division's monitoring inspection program. Mr. Miguel Hamarat, Mr. Tim Haynes and <br /> Ms. Amber Moran with Climax Molybdenum - Henderson Operations (Henderson) were present during the <br /> inspection. <br /> The inspection began with a meeting to discuss the ongoing permitting items at the site. The following topics <br /> were discussed: tailings delivery line structural support refurbishment and removal of Drop Towers 15 and 16 <br /> (TR-22), the completion of the first Ute Park extraction well, the status of the Ute Park extraction well <br /> infrastructure, the potential relocation of the seep water return line and the pending technical revision to <br /> remove Drop Tower 12 as part of the TDL refurbishment project. <br /> Tailing Delivery Line (TDL) structural support refurbishment and removal of Drop Towers 15 and 16 <br /> The removal of Drop Towers 15 and 16, the replacement of 297 linear feet of RCP with 42-inch HDPE pipe and <br /> structural support refurbishment of the Tailing Delivery Line (TDL) was approved by the Division March 27, <br /> 2014 under TR-22. Concrete footers were completed at Drop Towers 10 and 9. A formed stem wall was <br /> observed at Drop Tower 10. The continuing hydro excavation operations were observed up gradient of Drop <br /> Tower 6. The project is expected to be completed by October 2014. Henderson plans to submit a technical <br /> revision detailing and explaining the additional removal of Drop Towers 13 and 14 and the associated HDPE <br /> installation and the removal of Drop Tower 12. <br /> Ute Park extraction well and monitoring wells <br /> The temporary well casing for MLEX-1 was observed. The permanent well head and pitless adaptor assembly <br /> was observed in the mill parking lot. Henderson expected the excavation and installation of the vault and <br /> pitless adaptor to occur in a few weeks. Sections of discharge pipe were observed along the planned <br /> easement, but excavation for the pipeline installation had not occurred. No work on the required electric pole <br /> and line had occurred. <br /> Seep Water Return line relocation <br /> Henderson stated the remaining hill between 1-Dam and 3-Dam was being considered to be excavated to <br /> expedite the connection of the two dams into one continuous dam face. Currently, the seep water return line <br /> runs up the hillside and ends in the tailings impoundment. The excavation of the hill would require the <br /> relocation of the seep water line. The proposed alignment would result in the return line ending at the toe of <br /> the embankment and the return water would flow to the impoundment similar to the existing lead off lines. <br /> The relocation of the seep water return line and excavation of the hill will require Henderson to submit a <br /> technical revision to the permit. <br /> Additional Topsoil Stockpile Area <br /> The additional topsoil stockpile area approved under TR-17 was observed. Topsoil has been placed starting in <br /> the southwest end of the valley. The placement is proceeding across the valley from south to north and will <br /> continue to the northeast. Topsoil salvaging operations are continuing to the south along the east side of the <br /> tailings impoundment. Henderson estimates approximately 1 million of the required 4 million cubic yards of <br /> topsoil for reclamation has been salvaged to date. <br /> Page 2 of 4 <br />
