OPERATORS <br /> SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br /> WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br /> Possible Violation is indicated,you will be notified under separate cover as to when the Mined Land <br /> Reclamation Board will consider possible enforcement action. <br /> INSPECTION TOPIC: Availability Of Records <br /> PROBLEM/POSSIBLE VIOLATION: Problem:The Rules require Operators who are not continuously engaged in <br /> the extraction of construction materials must either file a notice of Temporary Cessation (TC) or apply for <br /> Intermittent Status (IS). <br /> CORRECTIVE ACTIONS:The Division recommends the Operator apply for Intermittent Status (IS) by submitting a <br /> technical revision for the site. <br /> CORRECTIVE ACTION DUE DATE: 11/21/14 <br /> OBSERVATIONS <br /> The Holsinger Gravel Pit was inspected by Peter Hays with the Division of Reclamation, Mining and Safety <br /> (Division)to follow up to the problems cited during the inspection conducted on July 16, 2013. Mr. Keith <br /> Holsinger was contacted prior to the inspection, but was not present during the inspection. <br /> The site appeared unchanged since the last inspection. The Division currently holds a financial warranty of <br /> $20,000.00, which appeared adequate based on this inspection. An entrance sign was posted as required by <br /> Rule 3.1.12. <br /> As stated in the previous inspection report, the pit has not been actively mined in approximately 20 years. <br /> Active mining operations require the development or extraction of a construction material from its natural <br /> occurrences, which may include; open mining, surface operations, transportation operations, processing <br /> Page 1 of 3 <br />
