PERMIT #: M- 2004 -061 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: August 7, 2014 <br />OBSERVATIONS <br />The Cottonwood Quarry was inspected by Peter Hays with the Division of Reclamation, Mining and Safety <br />(Division /DRMS) as part of the Division's monitoring inspection program. Mr. Richard Linke with Linke <br />Construction, Inc. was present during the inspection. <br />The Cottonwood Quarry is located approximately 2.5 miles southeast of Granby, Colorado on State Highway <br />40 and 2 miles west on Grand County Road 55. <br />The Cottonwood Quarry 112c reclamation permit was approved by the Division on December 10, 2004. The <br />approved mining plan indicates mining will begin on the southern part of the property on an existing natural <br />bench above County Road 55. Material will be removed from the natural bench until enough room exists to <br />move a processing plant into the mined area. Mining will advance to the north by creating 40 foot by 40 foot <br />benches. <br />A main scale house and stockpile bench has been created in the southwest corner of the site. The processing <br />plant has been moved to a bench northeast of the main bench. The active highwall consists of a lower 30' <br />highwall and bench area and an upper 35' highwall. Mined material is cast from the upper bench to the lower <br />mining bench before being transported to the processing facility. A haul road was constructed from the west <br />side of the hilltop to the upper mining bench. The portable asphalt plant observed during the last inspection <br />has been removed from the site. <br />The permit application anticipated blasting being required to mine the site. Mr. Linke stated blasting has not <br />been required since the rock has been fractured enough to allow mining to occur with an excavator. The <br />additional rock analysis suggested by the Division during the permit application adequacy review has not been <br />completed. Linke Construction agreed to complete the analysis when enough rock face had been exposed to <br />do a proper analysis and proposed limiting the mining disturbance to 20 acres until the analysis was complete. <br />The 20 acre disturbance was expected to occur after the first four 40 foot benches had been complete, which <br />has not occurred at the time of this inspection. <br />Linke Construction committed to placing a 6 -mil vapor barrier liner under the fuel storage area and up the <br />sides of the containment berm sufficient to contain any spill or leak during the adequacy review. Mr. Linke <br />stated the fuel storage area has not been required for mining operations and no fuel was stored at the site. <br />A storm water retention pond was located in the southeast corner of the site as required by the mining plan. <br />The Division currently holds a financial warranty of $84,073, which is adequate to complete the reclamation of <br />the site based on the observations from this inspection. <br />An entrance sign was posted as required by Rule 3.1.12. The southeast corner post and a brass cap in the <br />southwest portion of the site was GPS surveyed by the Division using a Trimble Juno 3B. A map of the survey <br />results is attached. Based on the survey, the mining disturbance is very close to the western boundary of the <br />site. The Division recommends the site is professionally surveyed to verify the mining disturbance is within <br />the permit boundaries. <br />Page 2 of 4 <br />
