1.0 Introduction <br /> This report presents a summary of new access road construction at the May Day Idaho <br /> Mine Complex located on County Road 124, Hesperus, La Plata County, Colorado. The access <br /> road was constructed by Opal Group, Inc. of Castle Rock, Colorado (Contractor). <br /> This report has been prepared as a attachment to the New Access Road As-Built Report. <br /> David S. McLay, PE, of DSM Engineering, LLC conducted field engineering and quality control <br /> during road construction and prepared this attachment. <br /> 2.0 Road Construction Summary <br /> Road construction started during the week of April 28, 2014. Mobilization was <br /> completed by April 30, 2014. The project kickoff meeting was held on April 30, 2104. The <br /> following briefly describes project milestones: <br /> • Week of May 18, 2014: Completed installation of erosion control measures (ECM)to <br /> protect the wetlands and La Plata River; installed tracking pad, removed soil berms, <br /> and installed ECM at the entrance to Colorado Route (CR)-124; and completed <br /> scarifying and recompacting road subbase from Station 10+00 to Station 16+50. <br /> • Week of May 24, 2014: Completed scarifying and recompacting road subbase from <br /> Station 21+00 to Station 27+29; completed wall foundation excavation; and <br /> completed tree removal. <br /> • Week of May 30, 2014: Completed installation of the pipe culvert and headwall at <br /> Station 19+12; and started construction of the concrete block retaining wall. <br /> • Week of June 6, 2014: Completed concrete retaining wall redesign to construct road <br /> at design grade; and continued construction of the concrete retaining wall. <br /> • Week of June 13, 2014: Completed construction of retaining wall. <br /> • Week of June 20, 2014: Completed installation of compacted fill for road subbase <br /> between Stations 16+50 and Station 20+00; completed placement and grading of road <br /> base from Station 10+00 to Station 27+29; completed installation of ECM and <br /> seeding at base of retaining wall, outboard side of road between road and the <br /> retaining wall, and the inboard side of road; graded and reseeded site of base of <br /> operations; and conducted final site walkthrough. <br /> The final site walkthrough was attended by representatives of the Colorado Division <br /> of Reclamation and Mining Safety (DRMS), La Plata County Fire Department, and <br /> neighbors of the property.
