PERMIT#:M-1997-011 <br /> INSPECTOR'S INITIALS:TC1 <br /> INSPECTION DATE:November 15,2013 <br /> OBSERVATIONS <br /> This inspection was conducted as part of the regular monitoring program. Mr. Joe Gaghano was present to <br /> represent the Operator during this inspection. This is a 112c sand and gravel mine. <br /> The active portion of the Baculite Mesa Pit is located approximately seven miles northeast of Pueblo off Baculite <br /> Mesa Rd. <br /> Inspection: <br /> The site was not operating at the time of the inspection. A permit sign was displayed at the site entrance (see <br /> Photo 1). The active portion boundary was marked with T-posts(see Photos 2 and 3). <br /> The highwall varied in height from about 8 feet(in the southeast) to over 30 feet in the southwest (see Photo 4). <br /> The excavation appeared to be proceeding in the south and west direction. Portions of the highwall exhibited <br /> considerable erosion (see Photo 5). However, eroded material settles out on the pit floor and did not appear to <br /> leave the site. For this reason, the eroded highwall was not cited as a problem. <br /> No distinction has been made between vegetative media for reclamation and backfill material. Material for both <br /> was observed stockpiled at the top of the highwall (see Photos 2 and 6). The only mine related equipment <br /> observed were a truck scales and scale house (see Photo 7). A small area on the north end of the pit has been <br /> seeded and shows some vegetation(see Photo 8). <br /> Records: <br /> • The total permitted area is 1,970 acres with a maximum allowed disturbed area of 40 acres. The 2013 <br /> annual report indicated 18.25 acres had been disturbed. <br /> • The previous inspection was performed on 8/7/2008. No problems were cited. <br /> • The anniversary date is July 9. Annual reports are current through 2013; annual fees are current through <br /> 2014. The Operator has promised the 2014 annual report will be submitted the first week of August <br /> 2014. <br /> • The post-mine land use is rangeland. <br /> Current Stipulations: <br /> • None. <br /> Bond: <br /> The DRMS has a $60,000 financial warranty (cash bond) for the site. The bond is considered adequate for the <br /> current disturbance. <br /> Summary and Recommendations: <br /> 1. Mining and reclamation appeared to be proceeding in accordance with approved plans. <br /> 2. No problems were observed. <br /> Page 2 of 7 <br />
