MCM Response: The specific Permit text discussion relating to the referenced wells has been reviewed and revised to clarify and update the status and use of these wells. Copies of the revised Permit discussion accompany these responses for replacement in the PAP. b. DRMS Response: The information regarding this well was revised and placed on page 2.05 -2. The revised page indicated Well DPP -94 -001 was plugged and sealed and released by CDRMS in 2012. A record of the abandonment of this well could not be located. Please provide the abandonment report for this well. MCM Response #2: Further review of the status of Well DPP -94 -001 indicates that it was not abandoned and that it still exists. The relevant text has been revised to reflect this correction and the revised page is included with these responses. MCM has identified several old wells that remain from previous owners /operations, and is currently developing plans to plug and seal these wells. d. DRMS Response #2: The response indicates that this well still exists and that the relevant text has been revised to reflect this correction. However, revised page 2.05 -2.1 now includes the discussion of well DPP -94 -001 and it indicates this well has been plugged and sealed as well as release from bond by the CDRMS. Please correct this statement on the revised page to reflect the actual condition of the well. 10. Permit page 2.05 -20 discusses topsoil replacement on the site and references Tables 58 -62, which are topsoil replacement tables. There are no dates listed in the footer for when these tables were created but, given the reclamation performed at the site in 2008 and 2009, the topsoil replacement volumes listed are probably not accurate. Please review topsoil replacement Tables 58 -62 and update the topsoil replacement volumes as necessary to reflect the reclamation that was completed at the Williams Fork Mines in 2008 and 2009. + a. MCM Response: The referenced soil replacement tables have been reviewed and updated, as appropriate, to reflect recent reclamation activities. Copies of the updated tables accompany these responses for replacement in the PAP. b. DRMS Response: i. Revised Table 59 lists the acreage for the 5A Portal and Pond 5A -P2 &3 under the "depth" column of the table. Please correct this table and indicate the depth the material will be placed over this area. ii. Revised Table 60 indicates the same volume of material exists in each of the topsoil stockpiles as is currently listed on this table and does not appear to account for the 2008/2009 reclamation of the No. 9 Portal and Waste Disposal pile. The revised permit text indicates this area was reclaimed and cover material and soil material was placed over the area. Please explain this discrepancy and/or revise the tables to account for the completed reclamation. c. MCM Response #2: The referenced tables have been revised to address the noted concerns, and the revised tables are included with these responses.
