August 1, 2014 C- 1981- 041/Roadside Portals MPB
length of west side of the home (see Photograph No's 5 and 6). The crack is approximately 30 feet long and 2
inches wide.
There was no indication of any cracks in the foundation although there is only a small portion of the foundation
exposed for inspection. We did not inspect the inside of the home.
This inspection report serves to document the features that Mr. Hasstedt has identified on his property within the
mine permit boundary that he believes to be potentially caused by subsidence from the Roadside Portals Mine.
HYDROLOGIC BALANCE - Rule 4.05
Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10;
Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13;
Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18:
Also during this site visit, I inspected the South Portal area. I arrived at the Outfall 016 at approximately 2:45 p.m.
The discharge from the outfall was measured at 311.2 gpm. I checked the two points of seepage at the portal. The
seepage below the concrete the wall is drying up; however there is some standing water from a recent rain storm.
The seep on the backfill material on the south side of the fill is no longer flowing. At the toe of the slope the
ground is just damp. The flow is contained to the french drain ditch. I measured the flow in the flume and it was
flowing at a depth of .09 ft which equals 4.53 gpm about the same as the last measurement on July 28th. The
seepage from the french drain system remains clear. The seepage behind the warehouse is less than the last
inspection and appears to drying up to some extent. No earth movement or slides have occurred to date. There
was no flow in the Fontanari ditches and they have completely dried up. The Carey Pond is also dry.
DOCUMENTS RECEIVED
None
OTHER (SPECIFY)
Number of Partial Inspection this Fiscal Year: 4
Number of Complete Inspections this Fiscal Year: 0
Page 3 of 6
