PERMIT#: M-1998-052 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE:July 17,2014 <br /> corrective action date. <br /> CORRECTIVE ACTION DUE DATE: 8/29/14 <br /> INSPECTION TOPIC: Signs & Markers <br /> PROBLEM/POSSIBLE VIOLATION: Problem: The affected area boundary markers are missing or incorrectly <br /> placed. This is a problem for failure to maintain boundary markers around the affected area as required by <br /> Section 3.1.12(2) of the rule. <br /> CORRECTIVE ACTIONS: The operator shall conduct a survey and replace the boundary markers in the correct <br /> location(s)—especially at the north and south ends of the permit area. The operator shall provide proof to the <br /> Division that this has been done by the corrective action date. <br /> CORRECTIVE ACTION DUE DATE: 8/29/14 <br /> OBSERVATIONS <br /> This was a routine monitoring inspection of the Mascot Placer 112c permit conducted by Eric Scott of DRMS. <br /> DRMS was accompanied on the inspection by Johanna Hopkins and John O'Hara of Everist Materials. This site <br /> is an 89 acre active 112c permit located along the Swan River approximately 6 miles east of Breckenridge on <br /> Hwy 6. This site is primarily a historic dredge spoil (cobble) removal project. Mining activity at this site <br /> consists of removing the historic dredge cobble and leveling the site without exposing the shallow <br /> groundwater near the Swan River. The reclamation plan for the area following the removal of the dredge <br /> spoils consists of spreading a thin layer of topsoil and seeding. Mining activity is currently taking place in the <br /> south 1/3 of the permit area. Although the mining activity is primarily localized to the areas of the historic <br /> dredge spoils, the permit boundary, especially the north, south and southeast boundaries will need to be <br /> clearly marked. <br /> No mining is taking place in the north end of the site, and it has been leased to a landscaping and nursery <br /> business. There are also two residential cabins in the north end of the permit, although they appear to have <br /> been there since the site was permitted. The permittee will need to submit a technical revision to the <br /> reclamation plan to account for the presence of the nursery within the permit boundary. The permittee <br /> should also consider at this time if it would be appropriate to seek the release of a portion of the north end of <br /> the permit area where the nursery and cabins are located. <br /> Two other issues were noted during the inspection that will need to be corrected. The first is insufficient <br /> secondary containment and stained soils at the refueling station area (photo 4). As stated on the first page of <br /> this report, this issue will need to be corrected no later than August 29, 1014 and proof provided to the <br /> Division that it has been done. The second issue is the presence of a make-shift gold sluice and pump near <br /> the center of the site (photo S). This activity is not permitted at this site and the sluice and pump will need to <br /> be removed. <br /> No issues other than the ones identified on the first pages of this report were noted. The corrective action <br /> date set at this time for the resolution of all the possible problems/violations noted is August 29, 2014. <br /> Page 2 of 4 <br />
