PERMIT#:M-1977-342 <br /> INSPECTOR'S INITIALS: PSH <br /> INSPECTION DATE:July 17,2014 <br /> OBSERVATIONS <br /> The Henderson Mine was inspected by Peter Hays with the Division of Reclamation, Mining and Safety <br /> (Division) as part of the Division's monitoring inspection program. Mr. Miguel Hamarat and Mr. Bryce Romig <br /> with Climax Molybdenum - Henderson Operations (Henderson) were present during the inspection. <br /> The inspection began with a meeting to discuss the ongoing permitting items at the site. The following topics <br /> were discussed: the financial warranty status, tailings delivery line structural support refurbishment and <br /> removal of Drop Towers 15 and 16 associated with TR-22, and the installation of the Ute Park extraction well <br /> and monitoring wells associated with TR-23. <br /> Financial Warranty <br /> Henderson is waiting for the Colorado Mined Land Reclamation Board (Board) to make a determination on the <br /> financial warranty structure proposed for the Climax Mine prior to submitting the revised bond estimate for <br /> the site. The Henderson site will request a similar financial warranty if the Climax warranty structure is <br /> approved. The Board approval meeting has been delayed. The item is expected to be presented before the <br /> Board during the August 2014 meeting. <br /> Tailing Delivery Line (TDL) structural support refurbishment and removal of Drop Towers 15 and 16 <br /> The removal of Drop Towers 15 and 16, the replacement of 297 linear feet of RCP with 42-inch HDPE pipe and <br /> structural support refurbishment of the Tailing Delivery Line (TDL) was approved by the Division March 27, <br /> 2014 under TR-22. <br /> Hydro excavating operations were observed down gradient from Drop Towers 10 and 9. The project is <br /> expected to be completed by October 2014. The Division has scheduled an inspection in September to <br /> observe the progress of the project. The Operator has determined Drop Towers 13 and 14 are not required <br /> and will be removed as part of the overall TDL refurbishment project. Henderson will submit a technical <br /> revision detailing and explaining the additional drop tower removals and HDPE installation above Drop Tower <br /> 12. <br /> Ute Park extraction well and monitoring wells <br /> The installation of extraction well, MLEX-1, and monitoring wells MLGW-21 and MLGW-22 were recently <br /> completed as described in TR-22. MLEX-1 was being cleaned and developed by repeatedly swabbing, bailing <br /> and pumping the well. The water pumped from the well was transported to a temporary holding tank, which <br /> will be pumped into the seepage collection system. Survey stakes for the future well locations, pipeline and <br /> electrical service were observed. The Ajax representative on site stated pump tests on the extraction well <br /> would occur over the weekend. <br /> Weed Control <br /> Weed control spaying operations were occurring at the site. The weed contractor stated the following area <br /> were treated: a topsoil stockpile near the west portal, the area surrounding the road relocation project at the <br /> north end of the toe of 1-Dam,the area surrounding the Ute Park well installation project and the road and <br /> area adjacent to the Ultimate Canal.
