PERMIT#: M-2009-024 <br /> INSPECTOR'S INITIALS:KAP <br /> INSPECTION DATE:July 9,2014 <br /> OBSERVATIONS <br /> This inspection was conducted as part of the Division of Reclamation, Mining and Safety monitoring program. <br /> The site is in the active phase of mining. An asphalt plant is located onsite and a pile of asphalt is stored on site <br /> to be recycled. The deepest excavation is on the west side of the site. Mr. Carnahan stated that at one point <br /> groundwater was encountered and the pit was backfilled to cover the exposed water. The operator now <br /> possesses a temporary substitute supply plan for the exposed groundwater, for future use. The temporary <br /> substitute supply plan is still valid for two years. <br /> No problems were noted during this inspection. A bond calculation was conducted as part of this review and a <br /> surety increase is included with this report. <br /> Page 2 of 4 <br />
