South Tavlor Fill Inspection Certification <br />On June 24th, 2014, 1 inspected the area in which the South Taylor Excess Spoil Fill is being <br />constructed. This report covers activities associated with both the East Taylor Fill and the West <br />Taylor Fill identified on Map 23A and Map 45 of the approved permit. The purpose of this <br />inspection was to observe status of the fill placement. This inspection is necessary to satisfy <br />the requirements of CDRMS Rule 4.09.10 (1a) and Rule 4.09.1 (1b). Weather conditions were <br />80 °F and sunny. <br />West Taylor Fill with A -A' and 13-13' Underdrain Area The toe is filled and sloped to the 7800 <br />elevation and rip rap placement has been completed to the 7695 elevation in the West Taylor <br />West Tributary and West Taylor East Tributary channels. Dumping continues at the 8050 <br />elevation of the Temporary out of Pit Fill. Topsoil is staged within a small area of the southeast <br />end of the East Tributary, with a ring ditch and berm constructed around it to prevent sediment <br />transport from the active mining area. At the time of inspection, topsoil placement was underway <br />between the elevations of 7715' up to 7775', approximately. The aforementioned topsoil pile <br />was the source for the topsoil placement. <br />The current water level reading of the piezometer well was 223.0 feet from the ground surface, <br />a decrease of 1.8 feet from the previous quarter. <br />No structural weakness, signs of instability, or other hazards were noted at the time of <br />inspection. <br />East Taylor Fill with C -C' Underdrain Area: The 7800, 7850, 7900, and 7950 dump benches <br />are completed. Since the last quarterly inspection, the Colowyo "025" Rock Crusher has been <br />moved to the flat pad above the East Taylor fill, and was placed at the 8000' elevation. Prior to <br />moving the rock crusher into position above the East Taylor Fill, the planned final rock crusher <br />location as well as the crusher fines spoil pile area was reviewed by Mark Gray from Shannon <br />and Wilson Inc., a Consultant for Colowyo. The crusher location and crusher fine spoils were <br />reviewed for compliance with the previous geotechnical study, titled "Revised, Addendum No. 3 <br />to Geotechnical Study for Excess Spoil, Temporary Spoil Stability, Job No. 23 -1- 01105 -206, <br />revised date April 15, 2010 ". After review of the planned crusher and crusher fines locations, as <br />well as the geotechnical study recommendations, Mr. Gray's opinion was that the proposed <br />"025" Rock Crusher relocation was in general conformance with the recommendations of the <br />above mentioned report. <br />Design requirements for the spoil pile are addressed in Volume 13, Exhibit 21, Item 1, <br />Introductory Letter. Inspection requirements during construction are further detailed in Volume <br />12, Section 4.09. <br />No structural weakness, signs of instability, or other hazards were noted at the time of <br />inspection. <br />
