Alternately, the Applicant may conduct all processing within the proposed <br />permit boundary without amending the Reclamation Permit application. <br />5. The original submittal identified three non - contiguous permit areas: Site A (0.75 acre), Site B <br />(1 acre), and Site C (1.91 acre). The revised snaps submitted in response to the Division's <br />Adequacy Letter dated July 2, 2014 show an increase in the affected acreage of Site C from <br />1.97 acre to 3.5 acre. The increase of affected acreage for Site C will require an amendment <br />to the application. Any amendment to an application shall constitute a new filing for the sole <br />purpose of determining the date for the consideration of the application by the Office and for <br />the deadline for a final decision on the application. <br />Alternately, the Applicant may remove Site C from further consideration under the current <br />Reclamation Permit application. If Site C is removed from further consideration, then all <br />maps and text which reference Site C must be revised accordingly. <br />6.3.5 Exhibit E — Map <br />6. Please revise the Mining Plan map to include the following: <br />a) Label the names of the owner(s) of record of the surface of the affected area and of the <br />land within two hundred feet of the affected area. If all surrounding land is owned by the <br />US Forest Service, then you may make a note of such on the map. <br />b) Outline and label overburden stockpiles. <br />6.3.6 Exhibit F — List of Other Permits and Licenses Required <br />7. The Applicant has stated a Plan of Operations (POO) will be obtained from the US Forest <br />Service. Please commit to providing the Division with a copy of the approved POO. <br />6.3.7 Exhibit G — Source of Legal Right to Enter <br />8. As required by Rule 6.3.7, please provide a description of the basis for legal right of entry to <br />the site and to conduct mining and reclamation for Owners of Record. This may be a copy of <br />an access lease, deed, abstract of title, current tax receipt or a copy of a BLM lode claim. <br />
