AVCOLORADO <br />Division of Reclamation, <br />Mining and Safety <br />Department of Natural Resources <br />1313 Sherman Street, Room 215 <br />Denver, CO 80203 <br />July 2, 2014 <br />Brady Miller <br />Miller Family Investments, LLC <br />10614 Hwy 66 <br />Platteville, CO 80651 <br />Re: Miller Family Investments, LLC, Lone Tree Pit, File No. M- 2010 -047, <br />February 14, 2014 Inspection Report — Unresolved Problem <br />Mr. Miller: <br />The above - mentioned site has one or more problems noted that, according to Division of Reclamation, Mining <br />and Safety (Division) records have not yet been resolved. According to our records, the following problem was to <br />be resolved on or before April 4, 2014. Listed below are a description and corrective action for the problem. <br />Please resolve the corrective action and provide evidence of such resolution on or before August 1, 2014. <br />Please note if the corrective action is not properly addressed, and evidence provided to the Division by the date <br />provided above, the listed problem could be escalated to the level of possible violations and would then be <br />referred to the Mined Land Reclamation Board for further action. <br />PROBLEM /POSSIBLE VIOLATION: <br />The mine identification sign was not posted at the entrance of the mine site. This is a problem for failure to post <br />a mine identification sign as required by Section 3.1.12(1) of the Rule. <br />CORRECTIVE ACTIONS: <br />The Operator shall, at the entrance of the mine site, post a sign which shall be clearly visible from the access <br />road with the following: the name of the Operator, a statement that a reclamation permit for the operation has <br />been issued by the Colorado Mined Land Reclamation Board; and the permit number. The Operator shall submit <br />photo documentation that a proper sign has been posted by the corrective action date. <br />If the problems have already been remedied, please provide evidence of their completion.
