PERMIT#: M-1980-131 <br /> INSPECTOR'S INITIALS: SJM <br /> INSPECTION DATE:July 25,2014 <br /> OBSERVATIONS <br /> This was a fuel leak follow up inspection of the Bunn Ranch Pit; M-1980-131 operated by Elam Construction, <br /> Inc.. This site is located than 1.5 miles southeast of Craig, Colorado in Moffat County. I, Stephanie Mitchell of <br /> the Colorado Division of Reclamation, Mining and Safety conducted the inspection. Mr. Gayle Lyman of Elam <br /> Construction, Inc. accompanied me on the inspection. <br /> This is a 133 acre 112c mining operation for sand and gravel, with a maximum disturbed area of 55 acres. A <br /> mine identification sign was in place at the entrance to the mine site and all permit boundaries were all clearly <br /> marked. Active mining was taking place at the time of inspection. <br /> The site experienced a diesel spill near the wash plant on June 10, 2014. It was reported to the Division and <br /> CDPHE on the very same day,June 10, 2014 at 5:15 P.M. The Division also received a formal report from Elam <br /> Construction on June 20, 2014.Thiis report was placed in the Division's files.The total amount of diesel lost <br /> was 92 gallons. The fuel leak spill area (18'x20'x 3.5' deep) was properly excavated and placed in a lined area <br /> for re-processing and burning of hydrocarbons in the future. The excavated area was properly bermed and <br /> upon approval from CDPHE, the excavated area will be backfilled with natural fines. <br /> During the inspection,there was a small area of the excavation area that still looked moist and had a <br /> faint fuel smell. A mini excavator removed more material from the area,totaling 4 cubic yards. <br /> Based on the observations and quick response from the fuel leak, Elam Construction did an excellent <br /> job of mitigating the situation.John O'Rourke from the CDPHE gave Elam the go ahead to backfill the <br /> excavated area and start up wash plant operations again on June 30, 2014. <br /> Photos showing the condidtions observed during the inspection are included at the end of this report. <br /> Responses to this inspection report should be directed to Stephanie Mitchell at the Division of Reclamation, <br /> Mining and Safety, Grand Junction Field Office, 101 S. 3rd Street, Suite 301, Grand Junction, Colorado, 81501, <br /> phone#970.242.5025. <br /> Page 2 of 5 <br />
