PERMIT#:M-1974-070 <br /> INSPECTOR'S INITIALS:PSH <br /> INSPECTION DATE:June 27,2014 <br /> Area 3 <br /> The entire section of land contained in Area 3 was flooded and scoured as lake#7 overtopped and flowed into <br /> lake #8. The center of the area, where a road was being established affected the most. The plan to repair the <br /> area consists to building a permanent road running east to west through the area and inter-seeding the other <br /> affected sections in the area. A technical revision will be required to update the reclamation plan to include <br /> the new road. <br /> The repaired area between lakes #6 and #7 was observed at the end of the inspection. Most of the repair <br /> activity was outside of the remaining permit boundary and was performed by Arnold's and Anadarko. <br /> The entrance sign and boundary markers were not observed as required by Rule 3.1.12. <br /> Photographs taken during the inspection are attached. <br /> Page 3 of 5 <br />
